The organization's name and a contact you can call. "Status as of" is the date the statuses were true; status can change, so an old date means ask for a fresh roster.
Each row carries the coach's role and the status word the organization recorded. The rule for which coaches appear on this document is being redesigned to match written guidance from the Florida Department of Law Enforcement, so this sample states none.
A named person at the organization generated it at a recorded time, and the platform logged that. This is the organization's attestation about its own coaches.
Open the link or scan the code. The platform shows the record it holds for that code: organization, date, coaches, statuses, and the document's checksum.
